Welcome to the Office of Certification’s application forms website. Below you will find a list of applications that can be sorted and searched for your convenience. Select “Instructions” under the application information category to review a detailed summary of requirements and information about each application and its processing. It is important to understand that while application requirement summaries are provided for your convenience, the summary information does not supersede authority and criterion of WVBE Policy and WV State Code.

West Virginia uses applicants’ social security numbers to check the clearinghouse maintained by the National Association of State Directors of Teacher Education and Certification (NASDTEC) for license revocation, cancellation, suspension, denial, and reinstatement in other states. West Virginia also reports information to the clearinghouse as needed. The West Virginia Department of Education (WVDE) will not release your social security number except to the NASDTEC clearinghouse to report cases of license revocation, cancellation, suspension, denial, and reinstatement as noted above.

Required Transcripts

Official, seal-bearing transcript reflecting all awarded degrees and/or courses required must be submitted. Copies are not accepted. Original transcripts may be mailed to our office, provided to county official where employed, or submitted as an eScrip (electronic transcript) sent directly from institution to our office to cert.transcripts.wvde@k12.wv.us. Electronic copies or PDF files forwarded from applicant’s email to the county, or our office, are not accepted.
